Your First Visit
We are happy that you selected Smiles by Design to be your dental care provider. We appreciate the opportunity to provide the best dental care available. We are able to achieve optimal dental care for our patients by using the latest techniques and materials in preventive, restorative, and cosmetic dentistry. We want your teeth to look good, feel good and function well.
Before your first visit,
please download and complete the following registration form. Because we are interested in the total health and well being of our patients, we ask that special attention be paid to the medical questionnaire. We need to be aware of any health issues or medications that you are taking. Doing so will allow us to immediately provide service to you.
Appointments
We try our best to schedule appointments so that you will not have to wait. We consider your appointment a reservation for the designated time. We appreciate 24 hour cancellation notice so that we may use the time to see emergencies. There is no charge for a 24 hour cancellation notice.
Emergencies
If you have a dental problem that needs immediate attention you will be seen promptly. When an emergency arises, please call as early as possible so we may adjust our schedule to accommodate you.
Fees & Payments
We make every effort to keep the cost of dentistry affordable. Towards that effort, we request that services be paid for at the time they are rendered. For your convenience, we accept cash, checks and major credit cards. Prior to extensive work, an estimate will be presented and financial arrangements made. Insurance
Most companies offer minimal employee dental benefits. Employees are often mislead, thinking they have comprehensive coverage, only to learn they only have minimal coverage. The average maximum yearly coverage offered by most insurance companies is $1000.00 to $1500.00, the same as it was 25 years ago. Make sure to talk to your human resources department to make sure you are maximizing your premium dollars. If you work for a large company, ask to look into a self-insured program. These programs won’t have nearly the amount of overhead as traditional insurance.
We will gladly assist you in preparing and submitting the necessary insurance forms. If you have any question regarding your dental insurance benefits, simple bring your benefit booklet to the office and we will be happy to spend time with you to review your coverage. In extensive cases, we will submit a pre-estimate to your insurance company so you will have knowledge about your non-reimbursable financial commitment. For reasons beyond our control, many insurance companies do not pay their claims in a timely manner. Therefore, any unpaid insurance balances older than 60 days will be subject to a 1.5% monthly finance fee.
